Existing illustrations have been updated, and new illustrations have been included, particularly where the text would be difficult to understand without them. References have been checked and updated – they are cited inline in a format that is brief yet still allows the reader to find the paper by typing it into a search engine. There has been a great explosion of literature that is also easily accessible, and further new material includes commentaries on what are deemed ‘landmark papers’ – these are not necessarily the most cited and include some from niche areas, particularly from difficult or emerging topics.
